Responsibilities

• Follow the home health Physical Therapy plan of care
• Coordinate and communicate with other disciplines involved in your patients' care, such as Skilled Nursing and OT
• Complete all electronic charting in a timely manner
• Identify and meet patients` goals and needs
• Reduce the need for medications and provide alternatives to surgery
• Develop care plans using a variety of treatment techniques
• Create fitness- and wellness-oriented programs tailored to patients` specific needs
• Provide quality, personalized and evidence-based care and proven interventions
• Motivate patients during treatment in order to help them function optimally
• Evaluate effects and monitor and communicate progress to Clinical Director and home health certifying physician

Skills

• Valid license to practice Physical Therapy in Arizona
• Graduate degree or greater in Physical Therapy
• Proven work experience as a physical therapist
• Competency with electronic charting and use of mobile devices, such as tablet, mobile phone and/or laptop/PC
• Patient and compassionate with excellent organizational and problem-solving skills
• Valid Level-One Fingerprint Clearance Card
• Current BLS certification required - Online certification is not accepted
