Seeking a committed and patient-focused Occupational Therapist to develop, recover or maintain the daily living and work skills of people with physical, mental or developmental conditions. You will offer effective and client-centered services that will enable people to confidently and independently carry out everyday tasks or activities. This is a PRN position that can turn into a full-time position with benefits.

Responsibilities

  • Provide skilled occupational therapy service to patients in their homes, in accordance with physician's orders.
  • Conduct initial and ongoing comprehensive assessments of the patient's needs, including OASIS assessments.
  • Select and adapt the appropriate per case occupational therapy treatment model, method and approach to direct the process of interventions.
  • Establish goals, based on the patient's current level of functioning, and potential for improvement and perform re-evaluations, as indicated.
  • Apply interventions to maximize safety and performance in activities of daily living (ADL) and instrumental activities of daily living (IADL).
  • Communicate with the physician, on a regular basis, regarding patient's status, obtaining additional orders, if necessary.
  • Evaluate the effectiveness of occupational therapy intervention and update the patient care plan, as needed.
  • Teach clients new ways of approaching tasks and educate them on how to break down activities into achievable components.
  • Assess client`s home and other environments and advice on alterations consistent with their needs and capabilities.
  • Recommend adaptive equipment and train in its use.
  • Guide and educate family members and caregivers.
  • Observe, note, and report on progress of long and short-term treatment goals.
  • Collect data and document processes followed.
  • Follow-up with client on a regular basis.

Skills

  • BS Degree or greater in Occupational Therapy
  • Licensed in the State of Arizona as an Occupational Therapist
  • Minimum one year work experience required
  • Home health experience required (SNF and ALF experience does not count as home health experience)
  • Familiarity with ADLs evaluation tools ( e.g. Katz ADL scale, Lawton IADL scale)
  • Competency with electronic charting and use of mobile devices, such as tablet, mobile phone and/or laptop/PC
  • Patient and compassionate with excellent organizational and problem-solving skills
  • Current BLS certification required - Online certification is not accepted
